Affidavit Instruction for Use
Affidavit Form is designed to guide users through a streamlined, five-step process for submitting affidavits related to Rezoning, Special Exception, Special Permit/Variance application. A green progress indicator shows your current step, and at various stages, you’ll have access to helpful tools like search and export-to-Excel.
In addition to new affidavits, if you need to review or make changes to an existing affidavit, use the Reaffirmation Affidavit Form. This form allows you to reaffirm, update, or revise previously submitted affidavit information. See Instruction for use.
Multistep Affidavit Form
The app walks you through a step-by-step process, with a green indicator showing your current position. At certain stages, search and export-to-Excel features are available for added convenience.
Affidavit Info: Enter the applicant’s name, affidavit type, and—if available—the PLUS number. Link to the PLUS app to generate PLUS number.
Title Owner, Contract Purchaser, Lessee: Provide the names and addresses of all Applicants, Title Owners, Contract Purchasers, Lessees and authorized entities.
Business Entities: Look for existing or add new individuals, business entities, and their relationships.
Business/Financial Relationship: Include any individuals who own 10% or more in the aggregate. Also disclose any members of the Fairfax County Board of Supervisors, Planning Commission, or members of their immediate households who may be associated.
Confirmation: Review all your application details. This is also where you can delete any entities. Make sure you have entered the affidavit signing date and the confirmation to enable the Submit button.
